For a copy of your medical records:

  1. Please click the link below to submit a medical records request; Do NOT submit your request in person. 
  2. There is no charge for your medical records.
  3. You should receive your records within five business days of completing your request.

Third-Party Requests:

Imaging CD Requests:

If you are a patient needing an imaging CD, please call any of the offices listed below for information on how to request, pay for, and schedule pick up of your copy of an imaging CD.
